Please take a moment to review the following information before completing your registration:

• Registration: You will not be charged at the time of registration. Your first payment will be processed either at your first appointment or on your chosen billing date, whichever comes first.
• Registration fee: Your first payment will include a registration fee per patient. Failing to pay on due date will incur a repeat registration charge
• Scheduling: New patients are typically scheduled within one week of registration.
• Insurance: We do not bill insurance for memberships. Memberships are typically for those patients without insurance. American Sun Care is not insurance coverage. Please review the Membership agreement
• Disability forms: Please note that no disability forms of any kind will be completed within the first 90 days of becoming a patient.
